Personal Exposure Reporting System

When Should I Fill Out The PERS Forms?

Toxic Exposure - The Hidden Hazard

When you answer a fire, HAZMAT or EMS call, the health risks of the job don't end when the call ends. During your career, you'll be exposed to hundreds of life-threatening chemicals and biological toxins. This is a fact of live for firefighters, but when it comes time to file an insurance or Workers Compensation claim, that fact sometimes isn't enough.

YOU NEED PROOF!

Personal Exposure Reporting

CPF's Personal Report System is the definitive system for reporting your exposure to cancer-causing chemicals and biological toxins. This unique database allows you to keep a secure permanent record of:

  • The date, time and nature of an incident
  • How long you were exposed
  • The chemicals and bio-toxins present
  • Any medical symptoms that appeared
  • Who was with you on the call

Armed with this information, you can confidently refute any challenge to your legitimate Workers' Compensation or disability claim, and give your personal physician a road map to on-the-job exposures that could help them provide a life-saving medical diagnosis.

With the CPF's Personal Exposure Reporting system, documenting your exposures is easy and secure. You can choose to submit your confidential reports on line or through the mail. All reports are integrated into the same data base. The result is the same, a file that will protect your rights.

CPF's new PER Online system (http://www.peronline.org) sets a new standard for secure, confidential recording of your hazardous and biological exposures. If you are more inclined to just send in the information, please contact the Local for your booklets. Mail them in, or send them to us, by US Mail, or by department mail, and we will forward them to CPF.
